Celebrate Independence is proud to provide the following menu of services: Community Development Services, Personal Supports, Personal Supports-Enhanced, Family Caregiver Training and Empowerment Services, Day to Day Administration and Respite.
Rates are guided by the DDA’s Reasonable and Customary Rate Sheet and all staff are employed per DDA’s staffing guidelines and requirements.
The following is a summary of the above-listed services, with all services being driven by the individual:
Personal Supports (Personal Supports Enhanced) support individuals in accessing opportunities to live their best life. Personal Supports are specific to the participant and their preferences.
Family Caregiver Training and Empowerment Services provide education and support to the unpaid family caregiver of a participant. Services can include education and training specific to the caregiver that will bolster confidence and promote agency.
Community Development Services provides participants with tools to develop and maintain skills related to community membership. This is accomplished through engagement in community-based activities involving people without disabilities.
Respite is intended to provide both the family and the individual a break from their daily routines. Respite services relieve families from their daily caregiving responsibilities, providing the individual with new opportunities, and experiences, and facilitating self-determination.
Day to Day Administration offers direct and non-direct support to participants through various tasks including employee scheduling, household management and scheduling, transportation scheduling and medical appointment scheduling.
